Occupational Therapist (OT) – Home Health

Alfa Care LLC is hiring Occupational Therapists (OTs) for full‑time, part‑time, and per diem roles.

✨ Retired or semi‑retired OTs looking for meaningful, flexible work are encouraged to apply.

Position Overview
The OT will help clients improve daily living skills, adapt their environments, and maintain independence through personalized therapeutic interventions.

Key Responsibilities

• Conduct OT assessments and create individualized care plans
• Provide ADL training, cognitive support, and home safety recommendations
• Educate clients and caregivers on adaptive techniques and equipment
• Document services in accordance with agency and regulatory standards
• Work collaboratively with nurses, caregivers, and case managers

• Active NH Occupational Therapist license
• Experience in home health preferred
• Strong problem‑solving and communication skills
• Commitment to compassionate, client‑centered care
