Physical Therapist (PT) – Home Health

Alfa Care LLC is seeking dedicated Physical Therapists (PTs) for full‑time, part‑time, and per diem opportunities.

✨ Retired or semi‑retired PTs who want to pick up flexible hours are warmly welcomed.

Position Overview
The PT will evaluate, plan, and deliver rehabilitative services to clients in their homes, helping them regain mobility, independence, and confidence.
Key Responsibilities

• Perform PT evaluations and develop individualized treatment plans
• Provide therapeutic exercises, training, and mobility support
• Educate clients and families on safety and home exercise programs
• Maintain accurate, timely clinical documentation
• Collaborate with interdisciplinary team members

• Active NH Physical Therapist license
• Home health experience preferred but not required
• Strong interpersonal and assessment skills
• Passion for helping clients achieve functional goals
